FESS was developed to solve the problems of Chronic Rhinosinusitis and designed to correct its pathophysiological changes. Over period of time surgeons began to use the same instrumentation to operate on other intra-nasal conditions like nasal polyposis and other benign masses in the nose.

Mucociliary transport mechanism of the nasal mucosa

Concept of key areas within the nose which are anatomical narrowings which if diseased, cause breakdown of the transport mechanism. e.g. Osteomeatal unit in the middle meatus.

FESS is a surgery aimed to solve the problems of Chronic Rhinosinusitis and associated patho-physiological changes.

Extended extra-nasal indications

The rhinologist has ventured beyond the confines of the para nasal sinuses and used the same surgical techniques for operating on the orbit, anterior cranial fossa and other intra-cranial pathologies. This newer development was also aided by the development of Image guided surgery.
